False PHD is a passive item introduced in the Repentance expansion of The Binding of Isaac: Rebirth. It functions as a high-risk, high-reward alternative to the standard PHD item. Pill Identification: Identifies the effects of all pills before you take them.
Immediate Drops: Spawns one random pill and grants one Black Heart upon pickup.
Negative Conversion: Converts all positive stat pills in your run into their negative counterparts.
Stat-Down Damage Buff: Grants +0.6 Damage for every standard stat-down pill consumed.
Horse Pill Scaling: Grants +1.2 Damage for every stat-down Horse Pill consumed.
Retroactive Buffs: Automatically calculates and awards damage for any stat-down pills you consumed earlier in the run.
Black Heart Generation: Drops a Black Heart on the floor whenever you consume a non-stat-down bad pill. Strategic Use & Synergies
Sacrifice Dump Stats: Trade non-essential stats like Luck, Range, or Shot Speed for a permanent flat damage increase.
Exploit Free Black Hearts: Use joke or situational negative pills (like Amnesia, R U A Wizard?, or Addicted) at the end of a floor to farm free Black Hearts safely.
The PHD / Lucky Foot Counter: If you acquire PHD, Lucky Foot, or Virgo, pills return to their normal good/bad item pool splits. However, you still retain the damage and black heart bonuses whenever you happen to take a bad pill.
Placebo Synergy: Combining False PHD with the Placebo active item lets you repeatedly trigger a specific stat-down pill to quickly maximize your damage multiplier.
